We currently have an opening for full time Service Coordinator. The Service Coordinator provides case management services to eligible clients. Will interview prospective clients, on-site or off-site, to determine their eligibility and assess their needs, prepare all necessary forms, with client cooperation, to begin the eligibility process and follow procedures to ensure swift consideration by government agencies of the eligibility of the case. Additional responsibilities include act as the individual’s advocate before review committees, assist with the provision of services after eligibility determinations are complete and prepare records to ensure timely billing for services provided.

A Bachelor’s degree in Health or Human Services Field is required. One (1) year experience with individuals with a developmental disability OR Master’s degree in Health or Human Services field.

A flexible schedule is also required for this opening.
